Thanks @adrianpearl for that lifesaving comment. We had this issue, and it turned out to be because of PgBouncer, setting `mode: session` fixed it

Parcel doesn't always generate index.html files in sub-directories

Observed same behavior, parcel `2.6.0` Upgraded to parcel `2.8.3` and the problem went away
