I have the same issue I'm trying to troubleshoot!

Do you have LDAP authentication enabled? I do - do you have an encryption key entered?

sorry for multiple posts - i managed to fix it although i'm not sure what i fixed exactly.... I first uninstalled module, than reinstalled. quick repair and rebuild rebuild config...

This happened again (4 months later) and had to regoogle it. so glad i put the solution in here... Rebuild config file through sugar!

whooops broke my click-to-dial - so i renamed the function myfix and it works now lol

What exactly does $e correspond to? Dial Prefix? Dialin Prefix? I know this is a stupid question.

This is obviously an asterisk issue... From the above looks like the callerID is the extension number. If your using freepbx there is an option somewhere in there for this.
