I don't have a meaningful example in mind right now, so maybe that's an edge case not worth considering, but since this is completely implicit, I think two Mastodon users...
One way would be to drop processing the *mention* (like with blocked people) on the receiving server. Other mentioned people would still be mentioned though. There's also nothing to signal...
I'm not sure what should be done there, to be honest. We can't really prevent the DM itself from existing, since people should still be able to mention other users…...

Ah, mastodon.online does not have Elasticsearch (the text search service) enabled if I remember correctly.
