The mod unfortunately isn't client only, it has to be on the server as well. If you put it on the server, the clients will need the mod too.

If I'm not mistaken, damage tilt is re-added to 1.19.4 on its own, so you don't need the mod anymore! Try removing the mod and seeing if damage is tilting...
