Unclear what was different this time, but I created another fresh project, enabled only OpenXR from the start (haven't imported any other plugin), and I am getting correct VR function...
It's still working this time after adding the Oculus package.
Things are working as expected this time around with OpenXR but not Oculus enabled, but I'm going to leave this open in case it happens to anyone else.
