Has this problem persisted? I have not been able to reproduce it yet.
I wonder if it could be a problem with the updater, though I'm not sure how to test it. You could try re-downloading the latest version of the client.
I tried downloading some old versions of the client & letting it update, but I still haven't been able to reproduce the problem. But there has been at least one...

This will be easier to implement for certain quests now due to quest manuscript format.
I didn't specify, & I don't remember for sure, but I think this is an issue with merging item stacks.
