bad blender version
because Unity loves to remove endbones sometimes. try connecting bones using cats to remove most of the issue
Yeah there is a way but it's very involved if you want to do it manually, but the jist is to find your blender addons location, take the mmd_tools folder...
This is an issue with blender 3.0 and 3.1. This is fixed in Development version/branch

dav64 is right. I use cats to help me with Neos which is wildly different from VRC
