Has it persisted through game restarts? Does a red background appear behind the ammo when your ammo gets low?

Unfortunately I don't have a 4K monitor and I don't believe it's possible to simulate higher resolutions in Source. Not sure what to tell you, I'm sorry.

Check that you're using the latest version and don't have conflicting mods. Haven't had this happen to me before.
