Consider to redact user picture when PR changes. E.g.: Title change, labels added, etc.

GitHub PR: username in document.title is not redacted

Just noticed that GitHub puts the username in `document.title`, and `blind-reviews` doesn't redact it:

From a first quick test, the author name is not redacted for inline (in-code) comments in GitHub. Version: 1.4.0 with Firefox
