Create a custom server for each user

If (and only if) allow_server_change is set, create a unique server per user (if no name is explicitly specified).

I think it would be nice to set a default per user tmux server name in instances where many users are using the same machine. Any reason this pull request was never committed?

FYI, I merged this pull request in my fork of wemux, which I started as @zolrath seems to have stopped maintaining it.
