XML namespace and readme improvements

Add XML namespaces to make SVG previewable. Improve readme instructions.

The current SVG fails to render in Chrome/Preview because it's missing the correct XML namespaces. I've added those and slightly increased the page size (purely cosmetic).

I also updated the instructions with a simple cp to install method (no need to manually edit an SVG file) and a screenshot showing where to find them in Inkscape.
