Improve the documentation and design of the cctx workflow

This EPIC tracks task to complete a revamped design for the CCTX workflow and management in the crosschain module.

The goal is:

Avoid duplicated code
Make the logic more future-proof and maintainable
Easier to understand

This is a requirement before completing WithdrawAndCall and IBC integration
