Agent Panel: agent perceives file as having changed when it hasn't
Summary
Agent thinks a file has changed and "re-parses" it, but the file has not changed
Description
I'm using Gemini 2.5-pro-preview (via Google, not GH Copilot).
At first, the agent just "re-evaluated" one or two files that hadn't changed. It was annoying after a while, especially as it started to occur in the middle of a response. For example:

But even so, it didn't refrain from the annoying behavior. Then, finally, after a few hours, there's this:

Hey @socketbox, thanks for the issue! For the sake of further understanding this behavior, has this been a consistent occurrence? If yes, is it a Gemini thing only or does it also happen with other models?
