I cam here to report a similar bug. However, I have never been able to get a consistent volume working with SmartTube, so I don't know if this is a YouTube change that has made the difference more pronounced or the "smart" volume functionality in SmartTube breaking.
The sound has definitely been substantially worse the last few days, but I have also never had consistent volume levels.
However, prior to today, I've never had videos on the extremes of the spectrum. Today, I've had videos that were functionally silent, even at max volume, and loud enough to make my ears ring, even when the prior video was at a reasonable volume.
If this bug isn't fixed soon, I'm going to have to find a different app. This is unusable.
but I have also never had consistent volume levels
This has been a longstanding issue for me with Nvidia Shield TV. My test for this issue is https://www.youtube.com/watch?v=euf-GKJV2S8 . Normally, the audio is very faint. If I go into Playback quality settings | Audio formats and choose 0.38Mbps, ec-3, 5.1, the audio becomes much louder. I hope SmartTube adds a default setting for audio formats akin to the existing one for video formats.
Volume is correct out of the box with the stock YouTube app (the new NoTube variant, actually).
I am also having this issue. Some videos just have super low volume and some just get lower and lower over time on long videos. I can temporarily fix the issue by going into the video quality settings and disable sound then reenable it and the sound comes back at normal volume (this only works for long videos .. videos that start super low are always super low)
Well I updated my daughters Chromecast With Google TV and the same thing is happening on that as well.
Still an Issue. It appears to have some relationship to the audio bitrate/codec/whatever. That would be fine if I had any control over what options were available.
I have the same bug. Even with audio level dropped to 5% within SmartTube the volume is very loud. On lastest Smarttube Update

Same here, lastest version on my 2015 Nvidia shield. Just done a complete fresh reset on the shield and it's still present. Audio level doesn't seem to do anything. I'm using a Denon x1600h, not sure if relevant
Volume to high on Chromecast HD, latest smarttube version. But since a few months now
