Additional request statuses

In additon to the existing statuses (no-reply, declined, success and partial) we would like to add the following:

• Online form: the organization has asked me to fill in an online form.
• Exsessive info: the organization has asked me to provide unproportional amounts of personal informtaion in order to identify me
• Provide a url: the organization has asked me to provide a url of where my data is located on their website.

In each case offer the individual the option of sending the company a custom reply.
