Enhanced support for multiple forum user <=>minecraft player links.

Right now, the plugin has dodgy support if the user wants multiple players linked to the same forum account, or wants multiple forum accounts linked with the same player.

These options should be configurable in the admin panel, and more obvious to the end user.
