Let `flac` signal location of errors

Currently, when flac encounters an error, it doesn't tell where it found that error. For salvaging and troubleshooting, it is probably very valuable though.

flac should say what it is expecting (metadata or audio) and how many metadata blocks or frames it has already processed.
