Add HeightLimits module for better configuration of y limits

Replace "PreventGoingBelowBedrockFloor" and "NetherRoof" with a more simplified and configurable HeighLimits module that allows per-world configuration and action configuration.

Possible Actions should include:

• Damage
• Kill
• Teleportation (+ possible safe teleportation option)
