Not working in Wordpress 4.2 (version 2.0)
I'm sure you're aware of it but Password Protected is not working with the latest Wordpress.
I have had a few reports, although it seems to be working OK in my development environments.
Are you able to give me any further details about your hosting setup? Who are you hosted with? Are you using any caching plugins? Does it have server-side caching (e.g. Varnish)?
Please would you be able to test this version of the plugin to see if that fixes your issue: https://github.com/benhuson/password-protected/archive/wp-update-3.zip
Download the above, rename the folder to 'password-protected' and replace the plugin on your site.
Just trying to narrow down possible causes...

Thanks. I just reverted a few changes that could have been possible causes.
Would you have a few moments to test this version (update 1): https://github.com/benhuson/password-protected/archive/wp-update-1.zip
...and this version (update 2): https://github.com/benhuson/password-protected/archive/wp-update-2.zip
... which will help me isolate the exact issue?

none of the versions posted here (update-1, update-2 or update-3) fix the issue for me... any ideas or ways we can help debug?

Could you try re-installing Password Protected 1.9 and checks that works with your WordPress install.
They were both released around the same time so it would be good to isolate wether something in WordPress 4.2 caused the issue, or wether it was one of the updates in Password Protected 2.0
Sadly, v1.9 didn't change things for me. I also tried deactivating all of my other plugins, resetting .htaccess, all with no luck. What really makes me frustrated is that about 5% of the time in the last couple of days it has worked for me—without making any changes on my end, and then it goes right back to the same issue.
My webhost is totally passing on helping me, might have to consider moving the site to another server...

Not helpful for other people but I solved this by moving to a different server (and hosting company). Never figured out the exact problem but clearly server-related somehow... Thanks again for your help.
It could well be your hosting had some sort of server side caching which may cause issues.
You might check again. I had the same problem with site on HostGator. Was about to try the updates as above, and it's working now, all I did was deactivate yesterday or day before and reactivate today. I have no idea if other updates made the difference or if HostGator made a change. I'd love to know.
It was working for me for a couple of minutes once or twice but never consistently and tech support was so utterly useless that I just decided to cut my ties. Still curious what the issue was but just happy to have it working...
Same issues, tried all of the update files. Made sure to reset my browser during each reinstall. No dice. Anything I can do to help? Your plugin is one of the cleanest WP password tools I've found, don't want to use anything else :(
