allow specify accesstoken

Some people are refused to log in to OpenAPI, and the following message appears: "OpenAI's services are not available in your country. (error=unsupported country)"

But as far as I know, the chat API does not have this restriction, so allowing users to specify their own accesstoken should help with this problem, like https://github.com/mpociot/chatgpt-vscode#instalation
