Make Alt-Q formatting key binding adjustable

The Format Paragraph key binding is Alt-Q, but is is hard coded into the plugin configuration and no option is aded to the key bindings page of the preferences.

Markdown View not updated without saving or leaving editor and returning

Update while typing doesn't work in the Markdown preview. You have to save your editor or exit it and return to see the new text in the Markdown View window....
