"an exception was thrown" is not consistently used

@domfarolino pointed out https://webidl.spec.whatwg.org/#an-exception-was-thrown to me, but this doesn't seem to be consistently adopted by this document. It's also unclear to me if this should be in Infra as that also has guidance.
