Unable to update a option correctly in radio button - Form Builder

What steps will reproduce the bug?

1. create a radio button in Form Builder
2. add two options [apple, orange] to the radio button in sequence
3. edit a label of the option orange to orange-2

What is the expected behavior?

the label of the option orange should be updated to orange-2

What do you see instead?

the first option apple was updated to orange-2 instead the option orange I selected

Additional information

1. no matter which option was edited, it always updated first option of the list
2. checkbox also has this issue
