The add button the Nested Form adds

I use this form and follow what the code for the Nested Form is. Create works flawless no bug or any error whatsoever, but when I try to update this issue keeps coming up. When I click on the add button on the inner table *(In my case the Item) instead of getting one field added, it adds 2 or 3 field at same time.
