D3 v4 update

D3 v4's selection immutability means that merges have been added when necessary. The name changes (e.g. d3.scaleLinear) have also been made. Comments noting most v3 -> v4 changes have been added inline, but these could be removed.

d3-selection-multi is used to show how the object map version works in v4 for the sake of completeness, but this style is now not recommended so a warning is added. Perhaps it is better to remove this slide.
