vllm Reshape cache to be XQA kernel compatible

XQA(GQA/MQA) kernel requires that the kv cache in a continuous GPU memory and in layout [num_tokens, 2, num_heads, block_size, head_size]. The provided reshape_and_cache_xqa kernel writes k and v into kv_cache.
