Are there any sites that have this that would be good to use for design inspiration?

Features related to the feedback button we can potentially build out:

• Stars for ranking
• Instantly send feedback without requiring log in
• Link to forum in the feedback section, as the answers may be there (from #635)

There is a nice animation on hover - moves slightly to the left and adds shadow:

There are faces for ranking the site. Text appears on hover for each.

After clicking a rating, a text entry window appears.

It asks for your email address after you submit feedback.

Last phase: displays a confirmation that your feedback was sent.
