Apex 18.2 support

Add apex 18.2 support. Review theme and changes in 5.1.13 section o apex 18.2 release notes:

this changes was in 18.1 too: https://docs.oracle.com/database/apex-18.1/HTMRN/toc.htm#HTMRN-GUID-4A0DC778-D262-4B64-8B1F-3C3A15319C92 im on 18.2 now, and material-apex forms items works fine. Actually changes from table to div markup is very old, i think they appears in apex 5 univesral theme.

all that i found for now is minor issues not related to apex version i think. This issues here, before your issue btw
