Document usage of custom patterns

Since #178, DiffKemp supports user-defined patterns of changes that should be considered semantically equal. Their usage is not very intuitive, though, so it would be nice to document the current state (how to write a pattern, what features are supported, how to use a pattern in DiffKemp comparison).
