Handle when the socket refuses connection.

So I had an issue when using this plugin with Semaphore UI for Ansible. First run would work but the second run the socket file would refuse the connection.

I looked through the Semaphore code for any weirdness regarding this but didn't find anything. I decided to handle the case of socket connection refused in the plugin.

Also cleaned up the markdown in README.md.
