Clarify how multiple rewrite rules are handled

It would be helpful to add some documentation regarding how multiple rewrite rules are handled.

The current behaviour makes sense, but it wasn't obvious (to me anyway) how the rules interacted, until I looked at the source code.

This PR is just a suggestion - I'm sure it can be written more clearly.
