Update to new appointment website

• added two new fields to config
  • mode and resetCheckFields
• update path to appointment date
• Note: Did not test for driver tests
• Note: There seem to be a few more fields that are used when scheduling an office visitj "numberItems": "3" (number of actions to take) "taskRID": "true" (Real ID) "taskCID": "true" (DL/ID) "taskVR": "true" (Register a vehicle) , but they did not seem to be needed, so I left them out
