fix module path

fixed the module path so v0.12 wont break on terraform init

Hi @torz. Thanks for the PR! We're working on a larger fix to bring our module in line with 0.12 We've also got some aligning to do with our other modules hence the longer dev time. The release will be done early next week. Cheers!
