Invoice: Form for billing co-location or other fees associated with resources

Many centers bill for the co-location of resources in their data centers or for the usage of shared equipment. For UB, we'd like to tie the resource attribute rack unit number into a billable form for invoicing co-location fees. We expect other centers to also want to include attributes for pricing of nodes to tie into billing as well.
