Add Microsoft.VisualStudio.ExtensionManager.dll for IVsExtensionManager et al.

Ah, this is in PrivateAssemblies, rather than the SDK. What's the strategy for this?

It works very similar to the current VSSDK.Language.CSharp.10 package.
