This commit expands visibility of failures related to auth tokens for upload / download endpoints and webshell via their respective apps in the following ways:

  1. Improve token manager responses to include reason why it fails
  2. Add middleware logger message on failure
  3. Add audit AUTHENTICATION failure in cases where relevant
