Not already assembled, but I uploaded Catskull's open sourced Arduinoboy gerber files to OSHPark, so you can order the board and DIY by soldering the needed components (and obviously assembling an Arduino Uno or any clone):

Also, I made a simplified board, named µArduinoboy (MicroArduinoboy), without PS2 and MIDI ports, compatible with Arduino Micro and clones, which is easier to assemble just by the fact that fewer parts are needed to be soldered:
