After Restart Prefix Title for some Windows is Lost... Have to Re-Save Title

There is a weird issue after restarting that after restarting firefox, the prefix title for some windows is lost (but other windows it does still exist). I have to go the title editor and the prefix is still there. I just have to click OK and then it re-enforces the prefix. It also sometimes just randomly comes back after switching tabs around. After it comes back and browser finishes the initial load (of thousands of tabs), it tends to stay and not disappear. So I wonder if this has something to do with cache?

This has been happening intermittently on every restart since 100+.
