table of contents: customization: layout, exceptions

Hi there,

first thanks for the great tool. I mostly use the merge function. About the table of contents generation, I think it would be really helpful to have the following features / improvements:

1. define a provided pdf page to be used for the TOC. This page has for example a given background and/or title.
2. TOC generate from documents titles: do not include a PDF in the TOC if no title is given
3. TOC generate from file names: do not include a PDF in the TOC if the file name includes the string "-notoc-"

The idea for the ToC generation is just to create a simple and handy way to navigate through merged files and as a general rule we try to keep it simple in options and logic, that's why it's missing a lot of options like font, size, color, etc etc for which a proper PDF editor seems more appropriate. Regarding 2 it would change the current logic (if I recall correctly) so I'm not sure, 3 is probably easy to implement. I'd wait to see if there is some user with similar requirements.

Yes this would be very handy. Also the ability to save a copy of the file list to a file so that it can be sorted / grouped in a different order.

I have seen this done in other applications using replacement of markers in a file foe example a Word doc eg {TOC} that picks up current formatting settinge
