This is an interesting one, because the tool arguably does a decent job of this, even though it doesn't faithfully replicate the original. Notice how it replaces the yes/no option...

The maximum output length is 4096 tokens. This, it seems, is only enough for JSON representing 2 or 3 pages worth of questions. I think perhaps the way to go...

I was playing with processing images of multiple pages, like this:  It only extracted the questions in the left column of pages 2 and 3.
