user-ss If a user selects north america, it may show zip code BUT not if they choose...China. i.e. Allow conditionally showing forms.

Implementation Options:

• Return HIDDEN/DISABLED from SQL query
• Return empty SQL query and toggle "HIde empty" or "Show empty as disabled"
