expunge dead nodes

If a node has not responded successfully for a set period of time, assume they are dead and remove them completely from the neighbors list.

Currently, they stay in the list but get passed over on read/write.

If the node comes back online at some point, they can ping us and get re-added.

Maybe track "dead nodes" in a separate list and ping infrequently.
