I feel that if I use underwear with shared top and bottom and clothes changed in the material editor, the pregnancy will not show up.

In the past I think It worked since I tinkered with material editor without any issues. If you enable the Preg+ debug logs under advanced options, it may tell you whether an issues is occuring.
