Update for Visits Registry

Hospital > Reports > Visits Registry

Suggestions:

• Fix naming: Menu name is "Visits Registry", report title is "Admissions Registry". These should be the same
• The action menu for each visit in the Visits Registry should include a "Discharge" link (but ONLY if the patient has not been discharged). This can currently be accessed via the "Patient Information" item in the action menu, but this is convoluted. It would be more straight-forward via the action menu.
