No longer works on my end
Been using this fix for quite a while, stopped working suddenly unfortunately. Might be the switch from 13.1 to 13.2. Invert horizontal and vertical scrolling no longer functions, even with detecting alternate methods.
Same, I'm on 13.3.1 (22E261)
Mac version. It doesn't work for this version.
I just installed it on 13.3.1 and it's working fine. Maybe try reinstalling it?
I had this issue and reinstall worked for me.
I will reinstall and test next week when I have a mouse to test against. Ty!

Oh yeah, I re-installed and everything works well
adding the command here just in case
brew reinstall --cask unnaturalscrollwheels
On 13.4.1, tried reinstalling, resetting, etc, checked perms, can't seem to get it to work. Any chance 13.4 broke it? Not seeing much buzz about this online though so I'm inclined to believe its an issue on my end.
@nicomiguel6 I'm on 13.4 and it's still working for me.

System Version: macOS 13.4 (22F66)
Kernel Version: Darwin 22.5.0
I have the exact same overview, have tried to reinstall multiple times. I noticed that the UnnaturalScrollWheels application does not even open when I double click it, nothing occurs whatsoever on my screen. Before I used to get a little icon in my taskbar that showed the app was functioning, now I do not... the app also doesn't show up on my Activity Monitor. I've allowed permissions multiple times, honestly ready to give up at this point.

Can confirm. Stopped working for me on 13.4.1 as well.
Can also confirm. Was working fine until the update to 13.4.1.

Works for me!
Make sure "Natural Movement" is enabled in MacOS settings, then these settings on the app:
