Certificate says no

The certificate doesn't display the full questions in a number of places

@ldodds do you have an example certificate for that, it looks like it's showing headings that only make sense when the answer is yes (and yes is replaced with a meaningful sentence, i.e in the provenance case the yes is replaced with 'machine-readable')

An example is here. Looking at the questionnaire it seems in some YesNo questions only the yes display value is set e.g. <yesno yes="some value" /> in the absence of no="something" then (my guess is) "No" is displayed on the certificate.
