Require a licensing answer in US jurisdiction

Currently the initial branch of the licensing question for the US jurisdiction is not required.

This means a Raw certificate can be obtained without indicating if the data is Public Domain or Openly licensed.

Both branches of the question once answered are required so I think the simplest step is to make this question required.
