Add reason to leave loop

@paloeka Could you add a list of options here of reasons to leave the loop?

• The current idea is: a pop-up appears when somebody leaves a Loop or deletes their account permanently (as they are also leaving a Loop in effect). Multiple answers are able to be checked in the pop-up. After selecting certain answers, a second pop-up needs to appear for a follow-up question. Be aware that multiple follow-up questions are able to be selected.
• The design template for DPA/ToH can be applied (if that helps)
• The pop-up is only when a participant {user} themselves leaves a Loop, a host can remove a participant without the pop-up
• Make sure that there is another additional pop-up that states that at least one answer should be selected, if people just want to send the form away
• The button to send the answer to the pop-up should be: Leave Loop

• Question/Prompt: Please share with us why you want to leave [Loop Name] Answers [multiple selection possible]:

1. The next address is too far away
2. I moved
3. I didn't find enough items I liked
4. I didn't connect to the Clothing Loop community
5. I didn't mean to register with this Loop
6. I'm done swapping

Follow-up questions: 2. I moved: [one option possible] a. At my new address, I plan to join a new Loop b. At my new address, I plan to start a new Loop c. I don't plan to participate in a new Loop anymore

3. I didn't find enough items I liked: a. The quality of the clothes didn't match my standards b. The sizes of the clothes didn't match my size c. The styles of the clothes didn't match my style
