feat(docs): issuer fees when same token

This is a proposed spec and docs change to add a 30 bp fee that can be claimed by issuers when the userToken and validatorToken are the same.

This ensures that validators always receive exactly 99.7% of fees, which means they are (at least in the ordinary case) indifferent to the fee token that the user uses. In the current behavior, validators receive more in fees when users use the same fee token.

It also helps set up for a possible future change in which issuers receive fees whenever their token is used as a fee token, and for other possible changes where fee payments might use multiple feeAMM hops.
