Add links to official website in readme.md for mentioned technologies/libraries

Readme file has technologies mentioned which will be used in Polaris. Need someone with markdown skills to put links to all the official websites of the docs.

I see you have merged @abhishekmayank 's request, and he has also claimed the other open issue. I'll find another repo to work on.

@danfoy there are more issues on this repo as well to work on. However, since he has claimed the open issue first I gave him preference of merging. I am posting few more issues, keep a watch, you might wanna pick something up.
